Husn Al-Zhann, Islamic Practices, and Mood Stability in Bipolar Disorder: A 7-Year Study
Abstract
Achieving long-term mood stability in bipolar disorder is challenging. Although there are effective pharmacological therapies for acute episodes of the disorder, mood variability, along with other symptoms, persists in many individuals, indicating the need for self-regulation processes in everyday cognitive-behavioural systems. The aim of this study was to explore the longitudinal association between mood stability in bipolar disorder, Husn al-Zhann (positive cognitive-spiritual orientation), and Islamic religious practices. This study used a single-case experimental design with monthly repeated measurements (n=82, from April 2019 to January 2026). Time series analysis and visual trend analysis along with non-overlapping indices were used to measure associations in subjects for YMRS, MDRS-22, Husn al-Zhann, and persistence in prayer and Qur'an recitation. The severity and variability of manic and depressive symptoms in this study decreased over time. Significant increases in Husn al-Zhann and non-overlap from baseline to the end of the study in adherence to religious practices were predictors of symptom stabilisation. Structured religious practices and cognitive-spiritual orientation may be considered additional self-regulation components for improving long-term mood stability in culturally sensitive bipolar care.
